For Paper Medical Records:

There will be a charge of 25 cents/page for the first 10 pages, then 50 cents/page for subsequent pages and capped at $50 to cover the cost to searching, printing and postage.

For Electronic Medical Records:

There will be a flat processing fee of $15.00. Medical record will be sent to a gmail address in encrypted format. Only the owner of this gmail address will be able to review the file for a defined period of time and file may not be printed or saved. If you desire and give us permission to send your medical record without encryption, please be sure that you are the only person opening the file.
