For Paper Medical Records:

There will be a charge of 25 cents/page for the first 10 pages, then 50 cents/page for subsequent pages and capped at $50 to cover the cost to searching, printing and postage.

For Electronic Medical Records:

There will be a flat fee of $6.50. Medical record will be sent to a gmail address in encrypted format. Only the owner of this gmail address will be able to review the file for a defined period of time.
